All of the lessons I offer can be purchased easily and securely online using your Debit or Credit Card or PayPal account. Once purchased the voucher/s will be sent to your billing address. Vouchers are to be redeemed in exchange for Golf Tuition, are non refundable and to be used within 12 months of purchase.

Courses of 6 lessons are the price of 5 when purchased in advance.
If you prefer to pay by cheque, please print this order form, and send it along with your cheque made out to "David Cook". Instructions and address information are on the form. I can only accept cheques in UK pounds sterling.
